Patient Centred Hub-and-Spoke Model
AlphaScript Pharmacy forms the nucleus of the Patient Centred Hub-and-Spoke model;
- The patient is referred to the licensed ACC Inc. clinic where a Health Risk Management Assessment(s) is performed, recorded and claimed through the AlphaNurse CMS.
- The AlphaNurse Clinic Practice Management System is fully integrated with the AlphaScript PMS as well as the AlphaDoc RCS which allows for the 2-way synchronisation of the Patient Journal and the electronic health records of the patients.
- The ACC Inc. clinic sister can during the wellness screening observe if the patient’s chronic disease is uncontrolled, refer the patient to his or her Doctor or dial-in to an AlphaDoc Virtual Doctor that can assist with a real-time virtual consultation, provided the ACC Inc. clinic is licensed as an AlphaDoc Clinic.
- The AlphaDoc Virtual Doctor, after making his/her diagnosis and updating the electronic Health Records of the patient on the AlphaDoc RCS, generates an e-Script which he/she wants to transmits on a real-time basis through the Alpha-e-Script Cloud Interface to the AlphaXpress web-browser used by the ASPN Pharmacy in which the licensed Alpha Doc Clinic where the consultation took place ,is based
- The ASNP process the claim response and prepare the medicine for collection by the patient on completion of the virtual consultation
- The AlphaScriptt pharmacy as the Central Chronic Medicine Dispensing provider schedules the chronic repeat cycles in the AlphaScript PMS and continues to supply the chronic medicine to the patient for collection at a time convenient to the patient from the ASNP.
- This is the first and only fully integrated real-time inter-referral system between a nursing sister , medical practitioners and a pharmacist in South Africa which not only fulfils our promise of becoming a leading Patient Centred Healthcare Group but also successfully reinstates the ICP as a primary care centre of excellence within their local community.
